Your Health Record

The surgery records details of your consultations, medication and letters confidentially on an electronic system called SystmOne.  This data is only shared with other organisations with your consent, although some relevant information may be shared with other health professionals when we refer you for further treatment.

There have been some recent developments to enable other healthcare professionals to be able to see your GP held record to improve the care that you may receive elsewhere.  A few years ago some of your data was uploaded to the Summary Care Record – this allowed hospitals and other health providers to be able to see (with your consent) some limited but important information such as significant illnesses, repeat medication and allergies.

Our system is now set up to enable us, with your permission, to share your full record with other healthcare services who  provide care for you.  These other services will ask your permission to view your record.  If you would like more information about this please take a leaflet, available in the waiting room.  If you decide that you would like your record to be shared in this way please let reception know your choices for use of your health data.

NHS England have also put a system in place to enable the NHS to use health information, sent from your record to a secure system, along with your postcode and NHS number, but not your name,  where it can be linked with other health information.  This allows those planning NHS services or carrying out medical research to use information from different parts of the NHS in a way which does not identify you.  If you have any concerns or wish to prevent this from happening, please take a copy of the leaflet “How information about you helps us to provide better care” or visit
